CLUB VIP Entertainment Card Terms and Conditions

By purchasing a CLUB VIP membership card, the purchaser consents to the following:

1. The card is personal to the member and NOT transferable to anyone else.

2. The card is good at participating locations only. Participating locations and card benefits are subject to change at anytime. Please consult www.247phoenixarizona.com for the latest valid locations or call CLUB VIP directly.

3. The card is good for 50% off the cardholder's personal bar tab. The discount applies to the normal, "Regular" price for any eligible drink. The card does NOT allow the cardholder to receive discounts on drinks that are already discounted such as happy hour pricing, 2 for 1 specials, or promotional offers. If a drink is already discounted, that discounted price prevails.

4. The card is also good for valuable discounts on any CLUB VIP limousine party fleet rental. See web site for details. In addition, certain locations may offer food discounts, line bypasses and other forms of VIP treatment for cardholders. See web site and e-mail bulletins for details.

5. Additional benefits or amendments to this contract will be added from time to time and cardholders will be notified via email of those changes.

6. Card must be presented BEFORE ordering to receive discounts.

7. The card may not be used to buy discounted drinks for anyone other than the cardholder unless the participating club has given permission for this purpose.

8. Each location has the right to exclude certain "Top Shelf" or high-end liquors, wines, champagnes and specialty items. For more information, please consult the web site or ask your bartender prior to ordering. Most mainstream drinks (well, wine, draft, call, martini, liquors, shots) are honored at most locations. Locations will vary and eligible drinks are subject to change. Ask prior to ordering.

9. You must be over 21 to purchase and use a CLUB VIP card. Each location has the right to refuse service to anyone. All arizona liquor laws apply at all times.

10. Cardholders should tip appropriately (20% is considered standard) on the price of the bar tab BEFORE discounts are applied. The price is discounted but the effort of the wait staff and bartenders is not. Please be an honorable VIP cardholder.

11. There are no refunds or free replacement cards issued for a lost or stolen card.

12. Each card is valid for one year from the date of issue and is clearly marked with an expiration date. Upon each cardholder's anniversary, the cardholder will be billed or charged for the next year at the prevailing rate at that time. Rates are subject to change and members will be notified of any such changes.

13. CLUB VIP reserves the right to terminate any member for inappropriate use of the card or for illegal acts of any kind. Such terminations are made without refund.

14. Any cardholders shall have the right to a full refund of their purchase price within 30 days of the receipt of the card. The card MUST be returned to CLUB VIP and thus retired from circulation to qualify for this refund.

15. If selecting our "Free Trial Offer", you are agreeing to provide a valid credit card for one full year of billing on a convenient monthly payment plan OR one single payment at the end of your 45-day trial period. After the trial period, your credit card will be billed for a $150 single payment or begin monthly payments of $20 on the first business day of each calendar month. To discontinue billing, your VIP CARD MUST be returned to CLUB VIP so it may be "retired" from circulation. 30days notice is required to discontinue monthly billing. Otherwise, billing will continue as long as that card remains in circulation. A card may be returned to CLUB VIP with a written request for cancellation within 45-day trial period with no penalty.

16. Arizona State law shall govern this contract.
